Chronicles Memory Care Program

Elmcroft’s memory care communities go above and beyond providing a comfortable, safe and nurturing home for loved ones with Alzheimer’s, dementia or other forms of memory loss. We call our memory care neighborhoods Chronicles, and we’re proud of the distinctive way we have created a community for these residents that meets their needs for meaning, purpose and independence.

What is Chronicles?

Elmcroft's Chronicles Memory Care Program features respectful and compassionate care of residents with memory loss. We strive to help all of our memory-impaired residents feel safe, secure and valued and help them live with dignity. Our communities feature three important distinctions:

Specially trained caregivers: Our dedicated staff receives specialized training in caring for those with memory loss. In this way, our residents are cared for as individuals, not patients. We’re here to take exceptional care of our residents, respecting their personal preferences and capabilities and being in relationship with each of them.

Our Best Friends® approach to care: A highlight of our program is our Best Friends approach to care built around each individual’s life history, cultural heritage, current abilities and interests. The Best Friends approach guides our caregivers in how to provide relationship-centered care to our Chronicles residents, just as they would with a beloved family member.

Secure and comfortable community: For our Chronicles residents, comfort and security are imperative for their health and quality of life. That’s why Chronicles features thoughtful design that includes secure indoor and outdoor spaces for the utmost in safe independence; décor that helps trigger pleasant memories; and multisensory elements to attract attention, encourage engagement and support relaxation.
